The assistants in my department are in the process of developing a survey that will be sent to managers and staff regarding the customer service the assistants provide administratively. Has anyone done this before? and if so, what type of questions should be included in the survey? Would greatly appreciate any feedback or suggestions.

I would start with the basics, such as asking how customers are greeted, how phone calls and messages are handled, etc. Once you have these basic questions written down, you can then expand on them - possibly give an example.
